Diagnostic/Initial Interview – $225
Individual Psychotherapy/Counseling – $150
Consultation – $150/hour
Clinical Supervision – $150 (60 minutes); $275 (120 minutes)
I am an in-network provider for BlueCrossBlueShield, United Health Care, UCare, Health Partners, Aetna, and PreferredOne. Please contact your insurance company to verify that I am in your network.
You may have access to out of network benefits if you have insurance through a different carrier. I am able to submit claims on your behalf even if I am an out of network provider.
Please keep in mind that using insurance means that I must submit a clinical diagnosis (if applicable). In addition, insurance carriers may require that I submit information about your treatment to determine if they will cover services.
You are responsible for co-payments/co-insurance at the time of service. If you are not using insurance, you are responsible for the full amount at the time of service. I accept cash, checks, and credit/debit cards.
Please bring your insurance card and photo identification to your initial appointment. Please provide updated insurance information if your plan changes at any point during therapy.
